Request a Police Report

You can request a copy of a police report in several ways:

Complete the Police Public Records Request Form.
Fax the form to: (928) 763-5558.
Phone: Call the Records Division at (928) 763-9200.
Email: records@bullheadcityaz.gov
Mail or In Person: Submit the completed form to the City Clerk’s Office, 2355 Trane Road, Bullhead City, AZ 86442.

Information to Provide

You may request a report in writing, by phone, fax, through an authorized individual with written power of attorney, or through a licensed attorney representing the victim.

Include as much detail as possible to help locate the report:

Case number
Names of people involved
Date of occurrence
Location of event
Your involvement (if any)
Name of any staff member you spoke with, if you called ahead

Fees

Victims may receive one copy of the report at no charge. You must be listed as the victim in the report.
For mailed requests, include a self-addressed, stamped 9 x 12 envelope.
