Police Records
Request a Police Report
You can request a copy of a police report in several ways:
- Complete the Police Public Records Request Form.
- Fax the form to: (928) 763-5558.
- Phone: Call the Records Division at (928) 763-9200.
- Email: records@bullheadcityaz.gov
- Mail or In Person: Submit the completed form to the Bullhead City Police Department's Record Division, 1255 Marina Blvd, Bullhead City, AZ 86442.
Information to Provide
You may request a report in writing, by phone, fax, through an authorized individual with written power of attorney, or through a licensed attorney representing the victim.
Include as much detail as possible to help locate the report:
- Case number
- Names of people involved
- Date of occurrence
- Location of event
- Your involvement (if any)
- Name of any staff member you spoke with, if you called ahead
Fees
- Victims may receive one copy of the report at no charge. You must be listed as the victim in the report.
- For mailed requests, include a self-addressed, stamped 9 x 12 envelope.
