Submit a Claim

The City of Bullhead City has established procedures for processing claims. All claims must be submitted in writing using the official Notice of Claim form. Forms are available below and must be filed with the City Clerk’s Office

  • Use the official fillable Notice of Claim form.
  • Provide three (3) estimates and/or supporting documentation.

  • Ensure all sections of the form are complete. Incomplete claims may be rejected.

  • The Risk Management Division reviews all claims for settlement.
  • Follow-up will be handled by the Risk Management office or Southwest Risk Services.
