• Three Coverage Levels

  • Preventive Care (100% covered): Routine exams, cleanings, X-rays, and fluoride treatments — no deductible required.

  • Basic Services: Fillings, simple extractions, and emergency treatment — subject to deductible and coinsurance.

  • Major Services: Crowns, bridges, dentures, root canals, and oral surgery — covered after deductible with coinsurance.

• Annual Maximum & Deductible

  • Low annual deductible (applies only to Basic and Major services).

  • The plan pays up to a set annual maximum per member each calendar year.

• Orthodontia (if included)

  • Orthodontic services for eligible children are covered at 50% up to a lifetime maximum, helping reduce out-of-pocket costs for braces.

• Costs to Employees

  

  • The City pays a substantial share of the premium for dental coverage.

  • Employees pay only a small monthly contribution (bi-weekly through payroll), which is much lower than market rates.

  • Preventive care is covered in full at no cost to you when you use an in-network dentist.
