Coverage Highlights

• Preventive Care (100% covered): Routine exams, cleanings, X-rays, and fluoride treatments — no deductible required

• Basic Services: Fillings, simple extractions, and emergency treatment — subject to deductible and coinsurance

• Major Services: Crowns, bridges, dentures, root canals, and oral surgery — covered after deductible with coinsurance

• Orthodontia (if included): Covered at 50% for eligible children up to a lifetime maximum

Annual Maximum & Deductible

• Low annual deductible (applies only to Basic and Major services)

• The plan pays up to a set annual maximum per member each calendar year

Costs to Employees

• The City pays a substantial share of the premium for dental coverage

• Employees pay only a small monthly contribution (bi-weekly through payroll), which is much lower than market rates

• Preventive care is covered in full at no cost when you use an in-network dentist
